Affidavit question

If we are not going to get off the ship in Cuba, what do we do about the affidavit? I don’t see any boxes to check for that.....just leave it blank?

Each cruise line have it's own travel affidavit for Cuba. You may want to get a copy or call your cruise line to check. I found this one for NCL:

https://www.ncl.com/ca/en/sdncl/sdsecure/dforce/CubaTravelAffidavit/PSTravelAffidavit

There is a checkbox for not getting off the ship:

OR I will not disembark the vessel while in Cuba to participate in any of the above referenced activities during this sailing

BTW, if you are not getting off the ship you shouldn't have to pay for the Cuba visa either. You may want to check to make sure your cruise line is not charging you for a visa that you will not be using.
