If purchased in advance of the voyage, the prices listed on the Cunard US web site are $40 per person for the coach transfers to either St. Pancras or the Victoria Coach Station.

Yes - let front desk know you want to self disembark  - keep all your luggage in your cabin last night - do not put it out
Once Capt announces ship is cleared you can get off first...
In the case for instance, of QM2 NYC arrivals you can be in a taxi at red hook terminal by 07.15am especially if you are US citizen.
For SOU arrivals you should be off from 07.30 - 08.00 depending on arrival time and clearance.

proviso is you must be able to manage all your luggage without assistance when disembarking off the ship
