Exclusion only on the Chase Sapphire claims form?

Does anyone have a blank Chase Sapphire Reserve credit card trip insurance claims form? 

 

I'm remembering something I read in the past week that I want to validate.

I thought it was on this blog

What I read was: 

that if you book a trip MORE THAN 90 days out, you are NOT eligible for [ an important part ] of Chase travel insurance.

 

Apparently this exclusion is not in the booklet, but it IS on the claims form.

IMO if it's not in the booklet it's not a restriction in the policy.

 

This is not to say that they cover events not listed, but a restriction such as that is material.

Go online to Chase Sapphire info. It definitely states the insurance is eligible for trips up to  days in 90 days length. I didn’t read ninety days out. Not to say it wasn’t there. 

Bummer for us as we had hoped to use its modest coverage for a long cruise.

I cannot answer the OP's question regarding a blank claim form, but I also see nothing in my Guide to Benefits booklet (10/2018) about booking more than 90 days out.  However, coverage appears to be limited to trips that do not exceed sixty (60) days.  Where do you see 90?  Here is an excerpt from page 48 of my booklet.  The 60 days also appears in several other places.
