How to get to Old Fort?

Or you can walk.  Not far.  But taxis are cheap to El Morro (the old fort) from the cruise piers.  I believe they run around $7 (certainly not more than $10) for a taxi up to 4-5 passengers and then $2 per passenger above 5 for larger parties.

This may sound flippant, but can I suggest that you take a look at a google map of Old San Juan?  The walk isn't bad, but a taxi up there will be really quick- up the hill, along the water, and you get dropped off right at the end of the path.

Since the walk there is more uphill, perhaps you can walk back and stroll through Old San Juan- it's cool to walk through such an old city.
