Where does one locate the form on the HAL website to complete for issues related to allergies?

Last time I was on, I recall seeing that info somewhere in the registration section.

I let my travel agent handle this for me.  And I created an email with specific instructions.  She is a bulldog about finding the correct person to get it and getting their acknowledgement that they have sent it to the ship.

If you sign into your account on the HAL site and go to Manage Your Booking then Travel Planning then Essential Documentation and finally Accessibility you can scroll down that page to Food Allergies where there is a link to the Special Requirements Information (SRI) form. It has a section about food allergies.
