Holy Smokes !! 😨I HOPE it's also payroll deductible for some poor slob who had to explain how and what in blazes happened !! Gonna be some 'finger pointing' in the boss's office tonight and the usual brown  substance is gonna flow down the chain and land on somebody's head!

Most (Carnival Corp's environmental compliance is a notable exception) of the maritime industry has adopted the ISM (International Safety Management) code, and has evolved away from the "blame culture" and now does not look to assign blame for incidents, but looks for ways to prevent the same incident from happening again.  If the crew are found to have followed all of the company's ISM policies, procedures, and protocols, then no blame will be apportioned to any of them.  Likewise, the same holds true for the pilot, if he followed the association's ISM procedures.  However, each entity, the cruise line and the pilot association will conduct investigations to find the root cause of the incident, and look to establish policies and procedures to prevent it from happening again.

That's refreshing to hear, Chief. That attitude (no blame assigned, just find out what caused the incident and FIX the problem) should be used by companies here.

Many do.  Those that are ISO 9001 certified are required to have a "Quality Management System", which in many cases includes a "corrective and preventative action plan", which investigates incidents not to assign blame but to improve the systems the company uses to ensure that the same problem never happens again.
