guardian form for minor on cruise ship with grandma

i am a grandparent taking 2 minor grandchildren on a cruise. i would like to have a generic form to complete that is not cruise line specific.

Curious as to why you don't want to use one that the cruise line provides.

Whatever form you use, be sure to verify that it is acceptable to the specific cruise line you will be sailing on.

princess does not require guardian form. i want it so i will be responsible for any ,if needed, medical decisions.

It usually has to be notarized.  Give as much detail as possible.

we had all the forms filled out.  custody papers, etc   they never looked at em once lol  they looked at their Birth Certificates, and passports and we were good to go.  I woudn't have traveled without them, but they never asked
