There is also another form you must complete and hand to the gate boarding agent. Note Alaska airlines does not allow ticket counter check-in until 3 hours before flight. If I remember correctly, it just affirms that you have had a COVID test and required vaccinations even though you have already been screened at the ticket counter. If your vaccination was done in Canada (or other countries with vaccine passports), you scan the QR code and it is immediately processed. If you were vaccinated in the US or a country that does not use digital vaccine records you upload a photo of the document/card and it pops up a message says it will be reviewed by an agent shortly. They may still ask to see the originals at the counter. I have found WestJet is very particular on the spelling of the name when it tries to match the vaccine passport to the flight reservation. When that happens an agent at the airport has to confirm the records match. Not certain if the US airlines are setup to do this electronically, I suspect it is more manual.
