Try reading the thread from the beginning, the very first post has a link to the loyalty form which weirdly is called an embarkation form!

I've filled in this form for 2 previous cruises - on the first we had a 2-4-1 voucher for the a la carte restaurant and on the second a free week wifi package.....better than a slap in the face!!!

Oh, and what drinks are available on the all inclusive package?

How much does the room escape activity cost?

I usually try and request it beforehand but this hasn't always worked - cue hilarious bed moving sketch at midnight of embarkation night!!!
