San Juan is a great place to explore on foot. There is also a hoho bus that will take you on a tour of attractions away from the city center.

It definitely won't be crowded. It's quite a long beach located quite a distance from the city.

There's quite a lot to see at Pearl Harbor, so if your time is limited, it's a good idea to prioritise your time:

You can catch a city bus from near the cruise terminal to Pearl Harbor, if you want to save a few bucks.

You might want to consider catching the hop-on hop-off bus. There is a bus stops right at the pier which is nice. You can also select the optional boat tour of Boston harbor. We enjoyed land both!

We did a Baltic cruise in June (from Amsterdam) and visited the same ports-of-call that you mentioned. The weather was perfect and the cities weren't too busy at that time of year. Hotels tend to be very expensive in that part of the world. The summer months might be particularly bad.

I love both Aix and Cassis. You can't go wrong with either town.
If you enjoy the restaurant/cafe/architecture experience, Aix-en-Provence is nice. If you want to enjoy some beach time, Cassis is the place to go. The boat tour from Cassis to the Calaques is spectacular, if time permits.

The Circumvesuviana Commuter Train from Naples Central Train Station to Pompeii takes just 25 minutes but you might want to take a taxi from the cruise port to the Naples Train station as it is quite a walk.

We took the hop-on hop-off bus from just outside the cruise terminal and it worked out very well indeed. In addition to hitting the downtown attractions, it had stops at Fenway Park, Harvard, MIT and other attractions north of the river. We also took the harbor cruise, which was included in the 24 hour fare. Very nice.

We also love Lucca. The train from Livorno to Lucca worked out well for us. It's too bad that the Lucca train station is located so far from the port. One really needs to take a taxi or shuttle from the ship to the station.

We stayed in an apartment for a week in Old Barcelona last spring, which we found and booked on Expedia. It worked out very well indeed and was less expensive than staying at a hotel.
The place we stayed at was a short walk to Franca Train Station so we were able to catch a train to the airport at the end of our stay. The taxi ride from the port to the apartment was inexpensive.

Venice is spread out across a number of islands. Thus, it's a good idea to become familiar with the various districts so that you can focus on a portions of the archipelago on each of your 3 days. Most of the major attractions are in the San Marco district.

Although Pearl Harbor might be busy, there is plenty to see. You can just prioritise what you would like to see.

Victoria BC is a beautiful city and very pedestrian friendly. I'd suggest that you take a stroll from the cruise port along the waterfront (Dallas Road) to Beacon Hill Park, walk through the gardens and then into town. Visit the Royal Provincial Museum and the downtown area. Return to the cruise port via the David Foster pathway.

English Harbour is located on the southeast coast of Antigua, far away from the capital city of St. John's, where the main cruise port is located.
Nelson's Dockyard and Galleon Beach are located nearby.

We visited Boston last autumn on a cruise and took the HoHo bus. The day package include a harbour cruise. There was a bus stop conveniently located right at the cruise terminal which worked out very nicely for us.

Yes, an early check-in for your cruise sounds like a great idea. The Waterfront district of Barcelona is not far from the cruise port and is a great place to spend some time. You can enjoy a great view of the port area from atop the Columbus Monument.

I'm not sure which ports-of-call you are visiting in Italy, but the ones we have visited were doable on our own, or with the help of hoho buses.
We visited Corfu Town on our own a couple of years ago and had no problem seeing it on our own. Although the walk from the cruise port to town is a little longish, you can catch a local bus if you like.

We did a RC cruise last October and really enjoyed it. The weather was pleasant, the colours were lovely and we only had one day of rain. Make sure to spend 2 or 3 days in Montreal or Quebec City at the start or end of your cruise. There's plenty to see.

There are a number of nice towns on the Island of Maui. Although Wailea is beautiful, it's not cheap. Lahaina is great to visit, but is not a beach destination. If you're looking for less expensive accommodation on the beach, consider Kihei.

The roads along the Cote d'Azur are always very congested. In addition to the Cannes Film Festival, there is a rotating strike underway by the railroad workers. This is probably a good year to be conservative with your excursion plans and just enjoy your port-of-call.

We've taken the hoho bus in Lisbon and it worked out very well for us. Many of Lisbon's attractions are spread out throughout the sprawling city.

You can catch a ferry boat from the Port of Naples directly to Capri, which is nice.
If you haven't yet been to Pompeii, it is definitely worth a visit! You can catch the commuter to Pompeii from the Naples train station.
I'd save the Amalfi Coast for another time. Although it is gorgeous, the road along the coast is windy and slow. You need more time to enjoy the towns of Amalfi, Positano and Sorrento.

If you take a tour provided by your cruise company, you won't need to obtain your own visitor's visa. Although this might be a little more expensive than entering Russia on your own, it'll be more convenient and less stressful.
