Definitely take your passport. It is easy to get lost or miss the sailing. Having your passport with you could head off a lot of headaches should you have some bad luck. It's the old 'ounce of prevention' thing.

By far the easiest way is to simply get a transfer from Your cruise line. Fly into London Heathrow; get your luggage and there is someone from the line right there at the baggage claim. You'll be taken to a waiting area until all scheduled pickups are there, and then taken to the ship.
I do feel I should tell you though, that you shouldn't expect a lot of your tours to happen. We had three scheduled; one to Bruges, one to Normandy D-Day beach and one to Windsor Castle on the way back to Heathrow. We only got the Bruges one in. The Normandy was canceled as only a few Americans were on board; Windsor Castle was canceled as the Queen was in town. I know of others that had only ONE chance to see Stonehenge and had their tour canceled. If seeing Stonehenge is something you want to do; stay a day extra if possible and take a bus tour. Get the after-hours tour so you can go inside the circle; otherwise you can't get real close.
