I always book a cabin close to the aft elevators. However, I might think twice if there were going to be a lot of kids on the ship as they like to gather near them to "play games" (i.e. jump on, press buttons and jump off, "race" between two elevators, or the ones that have figured out how to press the buttons so it won't stop on any deck). They, also, like to gather near the steps. We don't cruise during those times anymore when there may be a lot of kids so lately has not been a concern. Just remembering the old days when we did cruise with our kids having no choice with school schedules. We told them if we ever caught them doing those shintiyan's, it would be the stairs for them for the rest of the cruise (lol).

Simply put no.
I saw pipe/cigar smokers all the time n the smoking area outside on Deck 15. Didn't bother me but there was no one there enforcing them to move down to Churchills.

I don't think I've been on a cruise yet (50 plus) that I haven't seen a sign on the front desk saying it is a full sailing and no upgrades available (lol).

Never hurts to check what the hotel would cost you on the economy rather than through the cruise line. The only downside is you would perhaps have to make your own way to Southhampton as they might not let you just buy the transportation. Even factoring in the cost of that, it could be cheaper.
