Go the the USA Embassy site. There are lots of web site doing this form for a fee but you should not pay. The US site does it free and will give you all the information you need. Never had to have anything visa/Esta for any of the island visited. Sometimes they ask you have your passport to show at dock entrances mostly you just show your ships pass. We take a photocopy of our passports as you don't want to carry the real thing around. Immigration at the islands normally board the ship and go through the passanger lists as a formality but you are not called to be present and can go ashore whenever you wish to. Make sure you both have the Bio-metric passports - you know if you have this at it has chevrons and I think you have to have at least 6 months left after your return date.
