Can someone please post the breakfast room service menu that you fill out in the evening and have delivered in the morning?

I have the Breakfast hang tag but do not have a scanner. It is pretty much a continental breakfast.

Juices - Fruits - Yogurts - Cold cereal - Baked Bread - Preserves - Beverages

 

I am a late sleeper so I wrote in ham/cheese omelet, and bacon, and had no problem getting it in the morning.
