I have used a consent form for minors traveling with us before, but wanted to make sure that I have the right age that a form is needed. I believe it's for anyone under the age of 18, could you tell me if that's correct?

yes the age is 18, make sure the consent form gives you permission to sign waivers etc and to authorize medical treatment

Yes. Minor Consent Form needed for anyone under the age of 18, and must be cruising with someone 25 or older.
