We did this last Oct. and really enjoyed it. It's an hour or so bus ride but worth it. The walled town is really different from anything I have seen before. It sits on a high hill The walls are just a continuation of the hill. I was impressed by the town well that was so deep they used donkeys to bring the water out. To answer your questions yes you do get an hour or so to shop. The lunch was very good and in a restaurant in the main town. I think you will enjoy the trip. By train it takes about an hour and a quarter, from the train station in Orvieto there is a funicolare up to the town. There is a map here (http://gram.eng.uci.edu/~alberto/orvieto/maps/map2.html) that shows the relationship between the train station and the town. No train, and I doubt there is a bus, but I don't know that for sure. Rental car or car and driver is probably your only option from Civitavecchia.
