I didn't use the ATM in the Casino, since an attendant indicated that it charged $5 / transaction (since I didn't actually use it, I can't confirm it). The casino charges 3% if you get cash from your shipboard account. I tend to use ATM's in the ports - while most work, some only accept local bank cards (as in my experience with certain locations in Japan). I tried to get $100 to play in a 5 cent slot machine and they would only give me tokens or chip voucher for the tables. they said it was a Carnival policy and they wouldn't give me cash. I would make them give me 100 dollar tokens and then stand back in line and cash them in. How stupid is that. Shame on you Carnival and shame on the Princess for allowing Carnival to buy them out.
