Yes you do need to complete the card.

 

Bahamas: Depending where your going, we put "Nassau"

From what port: Usually port you set sail from: NY, NY

 

List your questions, you will find the answers here.

 

Read the card carefully, if you screw it up, there are always extra cards at the port in NY but you will need to get out of line to fill them out.

Could be rather crowded.
