Embarkation medical questionnaire

Do we still fill out a medical questionnaire at embarkation?

I really don't remember doing so last year when sailing on the Pride, but am pretty sure I did it for RCI, and on an earlier Pride cruise. I believe I even had the form printed off at home and filled out before getting to the port. If this is still required does anyone have a link to the form?

No, but we were asked, upon checking in at the port, if anyone has had cold or intestinal issues in the past 48 hours.

Surprised they are not requiring that they be filled out. We sailed Princess in Dec. (prior to their NORO outbreak) and the agent checking us in went over the forms and they seemed to be looking for cold symptoms. A lady checking in 2 stations over admitted to a cold and they asked her to move to the side to be examined by a nurse. I saw her on board, so she must not have been too bad, although I sure avoided her when I saw her coming on her scooter.

we weren't asked any health related questions last month. I guess we just look ridiculously healthy..:D
