Does anyone have a copy of the FLL/Southwest Luggage Valet form that they could share. I have heard different people give different time frames for what is the latest Flight Out you can have. I would appreciate it very much!

FYI: When I signed up for luggage valet on my last cruise, I discovered it will be denied if you have a Southwest senior fare. Seems you have to present yourself to an agent to prove your age.
