My wedding is quickly approaching and I'm just getting around to looking at these forms from the Office of Vital Statistics. For those who were married on NCL on embarkation day (ours is in Miami), did you have to fill these out? I saw the post about needing them for getting married in the Bahamas, but I don't really see why they would be needed for a wedding in the U.S. I asked my ROE coordinator and she said to fill them out and provide divorce decrees if applicable. Does this mean I only need to fill in the info and submit to ROE? I'm going to respond back to her with follow-up question but I figured that there will be someone on here who's been through the process. They sent us 3 forms; Application for Marriage Record for Licenses Issued in Florida, Application for Florida Birth Record and one for the Death Record. The Marriage Record form makes it seem like its for proof that you're not married in case you're being married in a different country, so I'm hoping I don't need to worry about it.
