Some run underground in the city itself but above ground once outside the center of town. There is Trenitalia, the state rail, Italotreno, a new competitor on the high speed lines (Naples to Rome, for example), the Circumvesuviana (more like a subway that runs at ground level than a traditional train), the Cumana and Circumflegrea (similar to the Circumvesuviana but running north of Naples) and other suburban lines. There is a fixed fare of €70 and it covers the historical center of town, as well as gulf/waterfront area and Campi Flegri. That might be enough to give you the overview you're looking for, and the price isn't much more than four HOHO bus tickets.
