US Dollars are generally accepted in the DR, exchanging is easy at any bank or hotel. I tend to use ATM's where ever I go, and have never had a problem including in the DR where I go about once a year. I would expect Tortola to be the same.

Can you use US currency in Tortola and the Dominican Republic, I assume you can in St. Thomas? If not, does the ship exchange US for local currency before getting off at these Ports?

 

St. Thomas is a US Port so US Dollars are the official currency. Also, US $$$ is the official currency of the British Virgin Islands so you are all set with US $ in Tortola. The Dominican Peso is the official currency of the DR but the US Dollar is widely accepted, no need to exchange your money. Enjoy your trip! :)
