Transfers from JFK

Does anyone know how the transfer works from JFK to New York hotels ,where do you meet at airport,and from hotels to QM2 .

When you have booked through Cunard

We had a Cunard pre cruise stay flying into JFK in July once we had collected our luggage and entered arrivals hall we very quickly found a representative holding a Cunard sign she then took us to the outside taxi pickup point and contacted we presume a pre booked cab which we had too ourselves to The Hilton Midtown 

Due too time of day we arrived around 5pm into NYC the transfer due to commuter traffic took roughly a hour to hotel.
