Insurance for WORLD cruise

Trying to find information about insurance coverage for a WORLD cruise, more than 90 days. Most plans I have found only cover you for 'up to' 90 days. Mainly need  coverage for trip interruptions, both for family (have older parents) or in case of an issue onboard, as well as medical and evacuation. My spouse and I are fairly healthy and do not have any extreme issues. The cost of the cruise is fairly steep and don't want to lose it, if something happens.
