DALLIEDOG Posted June 13, 2006 #1 Share Posted June 13, 2006 We are taking my granddaughter (age 7) to Alaska in August on the Norwegian Sun and we need to obtain a Notarized letter of Authorization from her parents (my daughter and her husband) to have at hand at all times--on the ship, Airport Customs -etc. Does anyone out there have an example or a form letter we can use to as a starting point as we draft this letter.. Thanks. Norwegian Sky 2002 Norwegian Sky 2003 Norwegian Sun 2004

zimmerjulie Posted June 13, 2006 #4 Share Posted June 13, 2006 That is the consent form that NCL wants for medical authorization & liability concerns for the minor. It is also advisable to carry an authorization letter something like this: http://www.singleparenttravel.net/Writing/Documentation/NotarizedLetter.htm for Customs & Immigration-type situations. I make sure to include the dates of travel in the foreign country. Here is one that is used for Canada: http://maeci.gc.ca/canada-europa/france/visas/autorisation_mineur-en.pdf Sometimes they don't even ask, but it is better to have them, just in case. Have a great cruise with your granddaughter!
Rare Smellytoes Posted June 13, 2006 #5 Share Posted June 13, 2006 Make sure you keep that letter until you get home. I took my 16yo brother on the Dawn in April. Had the letter signed by my mother and they looked at it at embarkation. I thought that I wouldn't need it anymore and almost threw it away. The customs official in Puerto Rico sent me back up to my room to get it. Thank God I didn't throw it away.
Cruiser from MN Posted June 16, 2006 #6 Share Posted June 16, 2006 Either Call NCL or go to their website and they have a form specific for this. It is called "Parent/Guardian Consent & Release Form". It covers all that you need.
