U.S. dollars are happily accepted in Cozumel, and you'll get your change in dollars as well. Most merchants will give you a better price if you use cash/traveller's checks as opposed to credit cards. Of course, this means you loose any protection your credit card offers. I've always paid with cash.

Bring plenty of small bills. Shop keepers and restaurants do not always have dollars on hand to give you as change.
