on my eTicket Boarding Pass it says to bring my ID, passport, boarding pass, and it mentions a COMPLETED HEALTH FORM...what is this? didnt get one sent to me and havent seen it online..anyone know? is it something they give you when you get to the ship?

What ship, what cruiseline etc???????? It may be this Noro outbreak and requires a questionaire or it might be a questionaire for some third world country you are visiting.........something along those lines........need more information.

The only health questionaire I have ever filled out by the way.......is after the SARS outbreak a few years back........

There is a Health Form listed in the Forms for Download in your Captains Circle Cruise Personalyzer. I did a search for 'Health Forms' here in the Princess Board and found a few posts. Some said they ignored it and filled out a form when checking in, some sent it in.
