Rare mdgary Posted January 26, 2007 #1 Share Posted January 26, 2007 We are planning on taking a 15 year old friend of our son's (16 year old) on our western caribbean cruise in June and wanted to know what documentation we need to take with. The friend's parent's are divorced and the mother has remarried and has full custody. We want to make sure we have everything we need well in advance. Letter from mother that has been notarized by a lawyer. Letter should state, "I, <name>, grant <yourname> permission to take <kids name> on a cruise holiday. She is authorized to take the child to the following countries: <list ports of call>. Any questions please contact me on <phone number> or my lawyer <phone number>. If she really has full custody but the father has visitation rights then he will also need to get a similar letter. That's what I have to do when I take my daughter on a day trip across the border to USA...and I'm still married to her mother. I had this confirmed with the USA border police just last weekend. There is a link here...I used this and edited it to take out "Mexico" http://www.advantagemexico.com/images/custody_affidavit.pdf I had my husband sign it cause we were traveling without him. So it would be the same circumstance. You are ALSO going to need a notarized medical release form giving you permission to make medical decisions for the child in case of emergency. AND don't forget to bring the child's health insurance card with you. I had a medical emergency onboard a cruise ship and needed the insurance card when we were medically transported to a hospital in the states.
