passport question

I am filling out our Funpass online. I am using my BC and I don't have a passport so I'm not familiar with them. My friend finally received hers so she called me and I filled out all the info needed for the funpass. (document #, country of issuance, and dates) I need the City of Issuance/Authority but she coudn't find a city listed anywhere. Where can I tell her to find this?

On her passport there should be a place that says "Authority". Mine says "National Passport Center".

On the page with the picture, to the right of the issue date and expiration date it says Agency. Mine says Miami Passport Agency. Hope that helps.
