Costa Rica shore excursions - Zip lines

We were in Costa Rica last year and were on one of the private tour excursions (can't really remember his name-Mario I think) and some of our group wanted to zip line and some wanted to go to the banana plantation. We chose to do the banana plantation but one other party split and half came with us and half did the zip line. We received a call half way through our excursion from the husband of a lady in our party that two of there group were injured on the zip line when they sent one of the teenage girls across and something happened to her rigging and they didn't know it and sent her dad across, who was a large guy, and he hit her going really fast and injured her and himself. Sounds like they had a heck of a time getting them off. Made me glad we decided not to. Ladies husband said they had some serious safety concerns with this zip line. We zip lined in Jamaica and had a great time. Just wanted to let you know, wouldn't want to hear of anyone getting hurt.
