How does the soda card work?

You wait for a waiter or bartender. There are no passenger-accessible fountains on the ships. For your second question: Both. If you don't have a glass or mug, they will give you whatever they have behind the bar or in the dining room. If you DO have your own mug, MOST bartenders will fill it up for you. The reason I put MOST in caps is because some people have reported that some bartenders have given them a hard time about filling your "Mega-Jug 64 Ouncer" on some cruises. Best bet in that situation would be to just avoid that bartender and also talk to their manager about it.
