Formal nights vary based on which itenerary:

Eastern route = 2nd and 6th nights

Western route = 2nd and 5th nights (Though there has been some recent discussion on whether the 2nd night on this cruise is 5th or 6th, reports from recent cruisers still are reporting it to be the 5th.)

 

In addition to Chops and Portofino, only other charges for food are at Johnny Rockets ($4.95 each + cost of milkshakes/sodas) and at Ben & Jerry's ice cream in the promenade.

 

No charge at Sorrentos.

Yes I was told by RCCL twice..I double checked that on the western it is 2nd and 6th....:)
