Assuming you're a US resident there are quite a few options as far as annual policies -- Travel Guard, Access America and a few others offer them.

 

They are either very light on trip cancellation/interruption coverage ($1000 or less) or lack this coverage altogether. Also be aware that if it does include cancellation/interruption coverage that's not a per-trip benefit, but total for the year. The reason for this is because the insurer has no way of knowing beforehand how many trips the client will take and the cost of those trips. If one client does a couple of three-night cruises they're going to be okay on the cancellation risk. But if the client plans on doing five 10-night cruises on Silversea at $8000 per sailing the insurer hasn't collected near enough in premiums to cover the potential risk of loss.

 

Also, for the most part the medical and emergency evacuation benefits can be much lower than the industry average when insured on a pre-trip basis. I'm looking at one plan from Access America and their medical coverage is only $20,000 and the medical evac coverage is only $100,000. Nowadays most plans give you at least $50K medical and $250K emergency evac.

 

That Access America plan will cost you $255. With many plans, up to age 70, you can buy four or five single-trip policies for the same cost and get better benefits.
