Parent Consent Letter for Kids Traveling

We are going on RCI Freedom of the Seas in Dec '10 to the Eastern Caribbean. My husband is divorced and has two kids (ages 14 and 15) who are coming as well.

 

If I understand correctly, he will need to have a letter of consent from their mom?

 

Does anyone have examples of how they worded the letter or is there a form? Does it have to be notarized?

Yes, and yes. Plenty of posters here have a link for the "notarized letter", but I don't. All I can say is that you'll need it! Kids are both under 18, so the notarized letter is mandatory. Happy cruising!!

This is what We used..

 

I was asked by my TA to have a letter so this is what I drafted...

 

As my kids dad is out of state. We are divorced and I am traveling with my mom. So I put her name in it also, in case something happens to me. I needed one for the kids to go with her to Costa Rica. They never asked for it. Since we are starting in the US and landing in Canada we needed it. Will see if they ask... I have it in my hand...

Also extend your travel dates on both end just in case... And be specific...
