Certified Letter From Parents For Daughter's Friend Traveling With Us??

Does anyone have a copy they've used that you could email me privately of a "notarized/certified letter" you used when you brought along a child not your own? Our teen daughter is bringing her friend along on our cruise next month and would like to make sure we cover ourselves with the wording in the letter for her parents to sign and have notarized. We will also have them include medical release info. Just wanted to know if someone had a letter that "works" already, so we didn't have to try and invent one!

I have one. I used it for our cruise in November when my daughter brought her teenage friend with us. Its very simple but has all info you need. Its in microsoft word so you can just amend it to your own specifications.
