If you are a US citizen, you can take a closed loop cruise (departs from and returns to the same US port) that only goes to places on the list. You do not have to have a passport, you can use an original copy of your BC and a government issued photo ID (most us Drivers Lisence).

 

However, if you need to fly (missed ship, emergency at home, etc), you would have to go to an embassy or consulate to get a passport issued.
