Born4Travel Posted August 29, 2012 #1 Share Posted August 29, 2012 I will be traveling with my 13yo cousin on a Eastern Caribbean cruise without her parents. What paperwork do I need to have in order before I go. (other than passport) Thanks.:)
Softtop67 Posted August 29, 2012 #2 Share Posted August 29, 2012 You need notarized en loco parentis form signed by both parents stating that they give you permission to travel with the child and make decisions for them if they are not reachable
notentirelynormal Posted August 29, 2012 #3 Share Posted August 29, 2012 In addition to the letter I would also have a copy of her medical card and permission to treat. What I did as well, I made a copy of my DS and DIL's passport showing their signature to go with the letter stating I had permission to travel with my grandkids. I'd rather be safe than sorry. Not sure what "en loco parentis" means but most here just type up a letter themselves stating the obvious. Name of child, parents, yourself, where and when you are going to and how you are traveling, then get it notarized.
Softtop67 Posted August 29, 2012 #4 Share Posted August 29, 2012 It is the legal name of the form you mention. There are many templates that can be downloaded from the internet and I find it easier to locate them if you use the legal term. It gives you the same rights as the parent during a specific period of time. Not only is it used for travel but very popular for baording schools and sleep away camps. Nebr.cruiser Posted August 30, 2012 #6 Share Posted August 30, 2012 Our cruise line had a form we could download for the notarized permission from parent's letter. (RCI) That way, we knew it was acceptable to them. I have used the forms found online as well.
