jtdlmc Posted February 9, 2013 #2 Share Posted February 9, 2013 I believe you are given the forms when you go to Camp Carnival orientation and can fill them out while listening to the presentation (which isn't long). Last time I did mine online and if I recall correctly one of the forms had to be printed out and handed in at orientation as well.
