Definition of "formal dress code"

Sorry to be ignorant but precisely what attire meets the definition of "formal dress code"? Is dark suit & tie for men proper? Is a tuxedo required? Does my wife need a long evening gown or is cocktail dress or cocktail pants suit appropriate? Thank you for the information.
