But still nowhere does it tell you how to join Costa club for repeat cruises.

Didn't you fill out the application on the cruise? Perhaps if you call them, they will send you one.

I dug around and found this:

 

All guests who sail are requested to visit the Costa Club corner onboard the ship and pick up an application for enrollment. A Past Guest program will be in place by summer 2004.

Be sure to return your application prior to disembarkation or by mailing it to Costa Cruises 200 South Park Road, Suite 200, Hollywood, Florida, 33021.
