Authoriation Letter

I will be cruising on the Freedom with my 10 year old granddaughter in July. I was told I needed a notarized letter/authorization from her parent for her to travel with me and also his permission to get her medical care if needed. Is there somewhere online to get a form for this I could use rather than having to get my lawyer to do it?

The "when" satisfies a criteria of Customs and Border patrol. I would not remove it personally.
