Does ANYONE know WHERE I can find the Form on line to Print Out?

We didn't fill them out online because for some reason, they don't have them available online. The form came with our documents and we filled them out before we left and they took them when we checked in.

That is correct. We also got ours in with our tickets. I was told by my travel agent that we have to have these filled out and ready to be turned in when we check in at the beginning of our cruise. Not quite sure why the special forms, but oh well....:)
