jemingway Posted April 4, 2004 #5 Share Posted April 4, 2004 Please note that it must be signed by both parents and each signature must be notarized. Carnival is not as difficult as Princess about this, but I suggest you take 3-4 originals. This way you can give one to the guy who's not too sure but needs the CYA and still have originals for the next one like him. Always remember to say that you talked to Jerald(or Nataly with a Y) at Carnival and he/she told you to do it this way. You need a name that's not too common, but believable when they start telling you that there is a problem.
