Old San Juan is pretty small- we can walk to El Morrow reasonably quickly.  Your mobility really depends on how you move your toddlers- carry, push, or walk on their own?

 

Old San Juan is a UNESCO World Heritage area, so, yes, see it.  The old city walls, the old gate, the old forts, old buildings, etc.

Be ready for some narrow sidewalks- being many hundreds of years old, the roads were laid out way before cars....  There are a few streets where there's a lot of space, and the Paseo de la Princesa is also wide all the way to the San Juan gate.  Good area to walk.
