We had first hand experience with this a few months back. We had to file first with Medicare, then Medicare supplemental, then NCL insurance. I found the NCL insurance through AON the easiest and fasted to deal with. Medicare and the supplemental plan not so much. Took months to get their denials. Keep all receipts.
