Has any cruise "failed" it's test cruise?

I have not heard of any ship having to do a second test cruise.  Has it happened?  If not, it certainly makes the CSO seem less onerous.  No, I don't want to argue about it being right or wrong, that is just one of the things that opponents said would happen, so I was wondering if it had.
