rlindholm Posted September 15, 2021 #3 Share Posted September 15, 2021 I had a similar issue. I just went to the pharmacy where I got the vaccine and asked them to change it. I was prepared with ID and SS card just in case they didn't believe me but there was no hassle. Carnival said as long as you have a "bridging" document that shows the reason for the name change you are fine.
