Health questionnaire

I haven’t sailed with NCL for quite some time so not up to date with how they do stuff. On other cruises we have had to complete a health questionnaire stating that we haven’t tested positive or been close contact etc. does anyone have the exact wording of these questions. Thanks

I don't have the exact wording, but sailed NCL in February and RCI in April. Both cruise lines asked pretty much the same thing, have you had covid, do you have symptoms, been exposed to someone, do you have a cough etc. All basic stuff.

When I cruised on NCL last month there were no paper health questionnaires.  Instead the person doing the check-in asked the questions orally.
