We will be sailing Millennium in October 2022 from San Diego to San Juan.  Have cruised many times but never embarked or disembarked anywhere other than US.  After disembarking ship and transfer to SJU, what additional requirements will we see?  As far as security and Covid requirements?

Puerto Rico is part of the US. Your flight back to Illinois is a domestic flight. You'll go through the usual TSA formalities and inspections just as if you were boarding a domestic flight in Miami, New York or Chicago.

There's one additional requirement when flying from Puerto Rico to anywhere else in the US. Your bags have to undergo an APHIS (agricultural) inspection before you can check them in for your flight as you are not permitted to bring many agricultural products and even some food products from Puerto Rico to the mainland US. Here's a list of examples from the SJU website:   http://aeropuertosju.com/pasajeros/usda/  .

The APHIS inspection lines are located near entrances to the terminal.

You'll still be dis-embarking in the US. No more hoops than any other domestic flight.   The ag inspection is an Hawaii thing too.
